RN MDS Coordinator
GLENDALE NURSING HOME 59
Hetcheltown Road, Schenectady, NY 12302 Glendale Nursing Home is accepting applications for MDS Coordinator. - Determine potential Patient Driven Payment Method (PDPM) for admissions
- Completing timely, accurate assessments, MDS & care plans as assigned
- Complete and assure the accuracy of the MDS process for all residents
- Monitor Case Mix Index (CMI) scores, looking for potential risks and/or changes that may affect reimbursement
- Initiating care plans and supporting activities as assigned
- Creating and distributing monthly care plan calendars in a timely fashion
- Communicates with health care team to create applicable care plans
- Completes supporting documentation as required.
- Protecting the confidentiality of Resident & Facility information at all times
Minimum Requirements :
A) Graduation from a regionally accredited or New York State registered four year college or university with a Bachelor’s degree in Nursing and two (2) years of experience in professional nursing activities, which shall have included experience with Health Care Finance Administration/Minimum Data Set, Prospective Payment System, Medicare Part A, Patient Review Instrument Guidelines and related documentation forms; OR, B) Graduation from a regionally accredited or New York State registered four year college or university with an Associate’s degree in Nursing and five (5) years of experience in professional nursing activities, which shall have included experience with Health Care Finance Administration/Minimum Data Set, Prospective Payment System, Medicare Part A, Patient Review Instrument Guidelines and related documentation forms; OR, C) An equivalent combination of training and experience as defined by the limits of (A) and (B) above
Additional Requirements :
Maintain active RN Licensure in Sate of NY throughout employment Understanding of Quality Measures Highly organized, professional & motivated Solid computer skills Excellent communication skills Should be friendly and a team worker
