Monday-Friday, either 8:00am-4:30p or 8:30a-5:00pdepending on the clinic volume. Rotating Saturdays (non-patientcare shift)

RESPONSIBILITIES

• Patient Care & Service. Promote and restore patients’ health bycompleting the nursing process; collaborating with physicians andmultidisciplinary team members; performing various treatmentprocedures; providing physical, educational and emotional supportto patients, friends and families; supervising assigned teammembers
• Planning & Communication. Develop and documentindividualized care plans customized for each patient’s uniqueneeds, with support from the interdisciplinary health team asneeded; maintain effective communication to convey patient healthstatus, treatment plans and progress
• Electronic Health Record(EHR) Management. Demonstrate proficient use of an EHR
• includingaccurate patient and provider documentation and communication
• Compliance.

Adhere to required department and system protocols,regulations (local, state, federal) and education requirements

Required Qualifications:

• Registered Nurse license in New YorkState.

Valid NYS Driver’s License Preferred Qualifications:

• 1year of nursing experienced preferred.
• Prior home health,clinical and direct patient care experience preferred.
