• ConductIn Person patient interviews and comprehensive physicalassessments.
• Oversee the implementation and ongoing assessment ofthe patient’s plan of care through the management of home healthaides, LPNs, RNs, and other caregivers.
• Communicate patientconditions and collaborate with appropriate providers to delivercare when patient needs evolve.
• Provide education to patients andfamilies on proper home health care procedures. Ie. Wound care, IVadministration, medication management.
• Work to decreasereadmissions by promoting preventative care and ensuring continuityof care. To qualify as a Registered Nurse Case Manager (RNCM) withus, you will need:

Licensure:

Current unrestricted license topractice as a Registered Nurse (RN) in the state associated withthis position

• Current CPR/AED/BLS/First Aid certification
• Reliable transportation to/from care sites and/or work locations.
• One (1) year of professional experience practicing as a RegisteredNurse (RN) in home health or similar setting; previous casemanagement/utilization review experience preferred.
• OASISexperience preferred.
• Practical trach and/or ventilatorexperience preferred, not required.
