Unlock yourpotential at Trident Medical Center! Job Summary and QualificationsAs a Registered Nurse for Admission and Discharge, the top priorityis delivering exceptional patient-centered care. They are dedicatedto ensuring a seamless and efficient transition for patients comingin and out of acute care hospital settings. Specialized assessmentskills help ensure that patients are placed in the appropriate careareas and discharged. We offer education and post-hospitalizationcare as necessary. Their main focus is advocating for patients,families, and caregivers while embodying the organization’s vision,mission, and values. The goal is to give an unmatched patientexperience and attain optimal clinical outcomes. What will you doin this role: Obtains a nursing assessment history, and ensuresproper inpatient care unit admission. Takes appropriate action whenneeded, such as communicating special circumstances to physiciansand/or care team or reporting indications of abuse or neglect.
Completes and facilitates the completion of required documentation,such as admission assessments, risk assessments, medicationreconciliation, discharge paperwork, and post-hospitaldocumentation. Teaches patients/families/caregivers about apatient’s medical condition, patient status, treatment plan,medications and possible side effects, and follow-up measures.
Translates complex medical terminology to ensure completeknowledge. Completes extra discharge documentation, education, anddischarge care for special patient populations, to ensurecompliance with core measures for discharge. Ensures patients/families/caregivers receive copies of discharge paperwork and anyother documentation needed, such as referrals for ongoing care,educational materials, etc. Coordinates with HouseSupervisors/Nursing Administration to prioritize workload andidentify greatest need areas to ensure smooth patient transitions.
Demonstrates a time-conscious awareness and ability to prioritize.
Distinguishes between low and high-priority activities.
Whatqualifications you will need:
