Hospice Nurse Practitioner 3.7 3.7 out of 5 stars 7410 Northside Dr, North Charleston, SC 29420 Company Overview The Hospice Nurse Practitioner provides hospice care through the evaluation and assessment of the health history, health promotion, diagnosing medical conditions, management of health problems by directing and developing the plan of care, coordinating and collaborating with patients, families and other healthcare practitioners. Under the supervision of a physician, the nurse practitioner visits patients, completes physical assessments, performs face to face assessments and evaluates hospice eligibility as assigned in consultation with the hospice interdisciplinary team. Summary Wren Hospice is seeking a compassionate and skilled part time Hospice Nurse Practitioner to join our team in Charleston, SC. In this pivotal role, the primary responsibility would be to conduct face to face assessments. You will provide high-quality patient care in hospice settings, ensuring comfort and dignity for patients during their final stages of life. Your contributions will be vital in fulfilling our mission of making every moment count for our patients and their families. Responsibilities Completes the hospice face to face visit requirements prior to the third and subsequent hospice benefit periods to assess hospice eligibility based on the hospice conditions of participation and the local coverage determinations. Provides clinical information to the hospice medical director for interpretation for eligibility for hospice recertification. Enter face to face visit in patient EMR. Makes recommendations to the hospice medical director regarding patient eligibility for certification and recertification for hospice care. Reviews clinical information, coordinates and oversees patient care in consultation with the hospice interdisciplinary team and other healthcare practitioners. Provides medical care, palliates and manages terminal illness in compliance with the state Nurse Practice Act under the supervision of a collaborating physician. Requirements Current Nurse Practitioner license in the state of practice. Experience in hospice or palliative care preferred. Strong knowledge of pain management techniques and end-of-life care principles. Skilled at establishing/maintaining working relationships with key staff, marketing contacts, and clinical staff to ensure thorough understanding of Wren Hospice services. Excellent communication skills for effective interaction with patients and families. Ability to work collaboratively within a multidisciplinary team environment. Compassionate approach to patient care with a focus on dignity and respect. Holds current unencumbered license in the state of South Carolina as a Registered Nurse and Nurse Practitioner.
