Position Overview:

The RN – Hospice Care provides compassionate, patient-centered end-of-life care to individuals in their homes. This role focuses on symptom management, emotional support, and coordination of services to ensure comfort and dignity. The RN acts as a case manager, overseeing individualized care plans and collaborating with interdisciplinary teams to support both patients and their families during the hospice journey.

Key Responsibilities:

Deliver skilled nursing care to hospice patients in their homes, focusing on pain and symptom management. Act as a primary case manager for a panel of patients, developing and implementing individualized care plans. Monitor and assess physical, emotional, and spiritual needs of patients, intervening as needed. Administer medications and treatments as prescribed to ensure patient comfort. Educate patients and families on disease progression, hospice philosophy, and care techniques. Coordinate care with physicians, social workers, chaplains, aides, and other hospice team members. Provide crisis intervention and emotional support to patients and caregivers. Maintain accurate and timely documentation in accordance with hospice regulatory standards. Participate in on-call rotation as needed to ensure 24/7 patient support. Ensure compliance with infection control, safety standards, and ethical hospice practices. Facilitate referrals and transitions as appropriate, including respite or inpatient care when necessary. Advocate for the patient’s wishes and support the family in making informed care decisions.
