Position Overview:
The GAFC (Group Adult Foster Care) Intake Coordinator is responsible for managing the intake process for new GAFC clients and developing strong referral networks. This role ensures compliance with MassHealth regulations while actively engaging with community partners to promote the agency’s services and expand its client base. This position is office based with regular travel to referral sites and client homes.
Responsibilities:
Intake Coordination:
Respond to inquiries from potential clients, families, and referral partners. Conduct initial screenings to determine eligibility for GAFC services. Collect and verify required documentation (e.g., MassHealth eligibility, medical records, physician orders). Maintain accurate and timely data.
Communication & Documentation:
Enter and submit assessments and Prior Authorizations (PA) for approval and renewals in accordance with program and payer requirements. Maintain detailed records of all intake activities and communications. Educate clients and families about program benefits, rights, and responsibilities.
Referral Development & Outreach:
Identify and build relationships with referral sources such as hospitals, primary care providers, community health centers, social workers, and elder service agencies. Represent the agency at community events, health fairs, and professional networking opportunities. Develop and distribute marketing materials to promote GAFC services. Track referral trends and provide feedback to leadership on outreach effectiveness.
Skills and Qualifications:
GAFC Experience
- Minimum 1-2 years of experience in home care, case management, or intake coordination and knowledge of MassHealth GAFC program requirements and MDS paperwork. Communication
- Have excellent verbal communication skills and active listening skills to evaluate patients and assess their care needs. Customer Service
- Use customer service skills to answer questions and guide patients through the admissions process. Computer Skills
- Work with computer programs to verify insurance coverage, access and fill out digital forms, and maintain digital patient files, all of which require proficient computer skills. Time Management
- Use excellent time management skills to schedule appointments for patients and manage multiple patients at once. Organizational Skills
- Know how to prioritize tasks and immediately see what could be done to improve a situation, a setting, or a project. Sales
- Ability to connect with patients and convince them to do what is best for them.
