The individual within this role will work a schedule of Monday – Friday 8:00 am – 4:00 pm.

General Purpose of Job:

Responsible for the implementation of life skill classes and program activities to meet skill acquisition goals outlined in participant Support Plan goals. Essential Functions and Responsibilities Oversee and lead classes in areas of skill acquisition including ADLs, self-care, money social/communication skills, computer skills and other training goals relating to skill development. Oversee and prepare lesson plans related to life skill classes and implement curriculums that align with participant Support Plan goals. Oversee the collection of all annual client documentation, entering of information into electronic system to ensure all required deadlines are met. Coordinate the setting of all meetings for clients (relating to Support Plans and IPP’s), including setting dates of meetings, coordinating with all parties and sending out of all communication documentation. Facilitate support plan meetings and maintains communication with waiver support coordinators, behavioral analysts, group homes, and guardians. Develop and maintain up to date list of due dates for all consumers to ensure regulatory compliance. Serve as a liaison for Quest in communication with families, service providers and support coordinators of all people served in regard to annual documentation. Coordinates questions, concerns, or requests from supports with appropriate members of facility management. Develop, monitor and report on skill acquisition progress as part of Support Plan process, including entering new goals into SP progress systems, writing progress notes, validating skills and continued evaluation of skill acquisition programs. Train and monitor Skill Instructor implementation of life skill programs both inside and outside of classroom setting. Ensure effective communication with all members of the team to ensure that all individual goals are accommodated and worked towards throughout the individual SP year. Coordinate with Behavioral Analysts on individual behavioral plan protocols in relation to skill acquisition programs and classroom learning. Serve as active participant of reactive procedures during behavioral crisis at facility as needed. Responsible for managing inquiries for new clients, scheduling tours, processing applications, obtaining consents, and handling related administrative tasks. Perform daily reinforcer delivery for designated participants. Report all incidents of suspected abuse, neglect, or exploitation to the Abuse Hotline (1-800-96-ABUSE). Follow all emergency procedures during a drill or in an actual emergency. Report to work during inclement weather to ensure adequate supervision and care of all individuals in the home. This may include sheltering in place during a hurricane. Attend, participate and/or conduct all required training and monthly staff meetings. Follow all Quest policies and procedures. Employees are required to report serious issues to the employee hotline or to a Quest leader under the See Something Say Something policy. Management reserves the right to revise the job or to request that other or different tasks be performed as assigned. Education and/or Work Experience 18 years of age or older required. HS Diploma required. Bachelor’s Degree preferred. A minimum of 2 years of experience in developing and implementing behavioral and/or training programs with individuals with developmental disabilities/functional impairments required.
