Description:
Essential Responsibilities 1. Admissions Accept responsibility for ongoing case management of individuals who have been admitted by the Assistant Director of Rehab into the Vocational Program. 2. Assessment and Planning for Individuals in Caseload Actively participate in the development of Individual Service Plans (ISPs); conduct annual assessments; conduct quarterly reviews and the updating and revision of information in those plans; prepare and monitor behavior support plans; collaborate with other providers in carrying out plans Lead goal-setting process for each individual. 3. Support Individuals in a Caseload up to 30 persons. Provide direct services as described in the ISP and including weekly face-to-face interaction with each client and consistent tracking of goal progress, behaviors, and work performance. Coordinate, monitor and evaluate EARS services, as required by ISPs; provide training to Floor Supervisors regarding positive responses to individual behaviors. Communicate proactively with other service providers identified in the ISP, as appropriate to enhance individual development. Report to the ISP Supports Coordinator Case Manager information that may suggest the need to revise the ISP; attend ISP Team Meetings on and off-site. Under supervision of EARS Medical Trainer, administer medications and/or support individuals in the self-administration of medications, as appropriate. Treat injuries (First Aid) occurring at vocational facility and arrange transportation of sick or injured participants to appropriate medical care. Follow EARS Incident Reporting policy and cooperate fully with investigations following such reports. Assist in the development of and implementation of behavioral intervention strategies. Perform such additional services as may be required to enhance opportunities for development of desired skills and behaviors among individuals in caseload. Accept temporary reassignments from supervisor in order to meet staffing requirements. 4. Evaluation and Placement of Individuals in Caseload Follow EARS program policies/procedures and meet Chapter 2390 requirements related to placement in a higher level program or into competitive employment. Work with staff to document vocational strengths and needs, make assessments related to potential success in higher level programming, and assist with referrals of disabled clients to the Office of Vocational Rehabilitation. 5. Documentation and Records; Supervision Follow EARS program policies and procedures, including use of approved forms and record-keeping procedures and meet Chapter 2390 requirements in regard to participant records. Ancillary Duties Attend regular meetings with supervisor and regular team meetings with other Program Specialists. Conduct occasional mini-trainings for individuals participating in the Vocational Program at EARS. Maintain all required certifications (fire safety, medication administration, First Aid, CPR); Complete at least 24 hours of training annually in matters relevant to position. Assist with supervision of participants during lunch and during breaks. Monitor participant attendance and provide timely information related to their transportation needs. Cover for other Program Specialists when necessary because of their absence.
Requirements:
Applicant must hold a Bachelor’s Degree or Associate’s Degree in a Social Services related field; with experience.
