Job Description
2nd Shift Direct Care Worker Description As a Residential SupportSpecialist, you will provide a supportive and safe environment in acommunity residential setting that fosters growth, independence, andcommunity engagement for individuals who have experienced a brain injuryresulting in physical limitations, mental health, and/or cognitivechallenges. Your role requires professionalism and collaboration withteam members in alignment with MHA’s Core Values, Respect, Integrity,and Compassion, while adhering to organizational policies, programfunding guidelines, and applicable regulatory standards.

Tues/Weds/Thurs/Sun 3p-11p Essential Functions Engage with participantsrespectfully and encouragingly to promote self-esteem and empowerment.
Assist, teach, or monitor participants in personal care, mealpreparation, household maintenance, shopping, and other daily livingtasks. Adjust support based on participant needs, including personalcare up to total care, transfers, and mobility assistance. Facilitatecommunity engagement by supporting access to social organizations,public transportation, counseling, educational institutions, and otherresources. Teach self-advocacy skills and serve as a liaison withhealthcare providers, family members, and other relevant parties. Assistwith financial matters, including money management training, budgeting,and banking, while maintaining proper documentation of expenditures.
Support community integration by facilitating participation inrecreational and social activities. Provide transportation forparticipants as needed, ensuring vehicle safety and completion ofrequired training programs. Assess individual abilities, interests, andneeds to contribute to their Individual Service Plan (ISP) and documentprogress. Respond appropriately to emergencies, including medical andcrisis situations, while maintaining communication with supervisorypersonnel. Report and document incidents of participant abuse or neglectas required by agency and regulatory guidelines. Assist with medicalcoordination, appointment scheduling, and medication administration inaccordance with MAP standards. Monitor participant health conditions,recording vital signs and health-related data as necessary. Complete allrequired program documentation accurately and within establishedtimelines. Maintain daily logs, data collection records, and shiftexchange reports, including money and medication counts. Report concernsor incidents to supervisory personnel promptly and document themaccordingly. Utilize agency reporting systems, internet-basedapplications, and other necessary software tools. Be flexible withscheduling and prepared to cover shifts as needed due to operationaldemands. Actively participate in team meetings, supervision sessions,and professional development opportunities. Attend required trainingsessions and maintain certifications, including MAP, CPR, and First Aid.
Exhibit professionalism, organization, and strong interpersonal skillsto support program goals. Required Knowledge, Skills, and AbilitiesValid driver?s license, sufficient automobile insurance, an acceptabledriving record, and access to a reliable vehicle during working hours.
Ability to work independently, manage time effectively, prioritizetasks, and collaborate with a team. Strong communication,organizational, and computer skills. Adaptability to participants’changing needs. Patience, compassion, and strong interpersonal skills.
Ability to pass and maintain certifications in medication administration(MAP), CPR, and First Aid. Must be at least 18 years old. RequiredEducation and Experience High school diploma or GED required; collegedegree in human services or a related field preferred. Minimum of sixmonthe of relevant experience.
