This includes maintenance of ongoing communication and coordination with the referral, family members, client, healthcare providers and intake sources for all clients. The Home Care Coordinator shall plan for all home healthcare service coverage from therapy, nursing, caregivers, and companions, while ensuring that the patient/family are receiving excellent customer service.

REQUIREMENTS

High school degree or equivalent required Bilingual required Proven experience in healthcare/home care coordination, customer service and or staffing Working knowledge of HHA Exchange is a plus Ability to communicate effectively with employee, patients and their families Ability to define problems and tasks, collect data and establish facts, take action and resolve issues!
