DUTIES AND RESPONSIBILITIES
Needs assessment. (Example of tasks: using required forms; determines through assessment client’s assets, deficits and basic needs; reviews needs on regular basis). Develops service plan. (Examples of tasks: contact provider source; write goals which may include personal grooming and health teaching; medication knowledge; financial aid; etc.). Implements plan by providing case management services. (Examples of tasks: monitor client condition; make home visits; assist client in securing needed services). Represents the needs of the seriously mentally ill. (Examples of tasks: establish liaison with other community resources; identify service gap; advocate for client). Basic Living Skills Instructor. (Examples of tasks: provides didactic, process and experiential instruction to clients).
