The ideal candidate will have a strong background in social work and experience in various areas such as workforce development, child welfare, and behavioral health. As a Case Manager, you will be responsible for assessing clients’ needs, developing personalized care plans, and providing ongoing support to help individuals navigate challenges related to mental health, addiction, and social services. Duties Conduct comprehensive assessments of clients to identify their needs and strengths. Develop and implement individualized care plans that address clients’ specific situations. Provide crisis intervention and management support to clients in urgent situations. Facilitate access to resources such as housing assistance (including Section 8), healthcare services (including Medicare), and workforce development programs. Offer addiction counseling and support for individuals struggling with substance use disorders. Utilize motivational interviewing techniques to empower clients toward positive change. Collaborate with multidisciplinary teams to ensure holistic care for clients. Maintain accurate documentation of client interactions, progress, and outcomes. Advocate for clients within the community and connect them with appropriate services. Qualifications Bachelor’s degree in Social Work or a related field; Master’s degree preferred. Proven experience in case management or social work settings. Knowledge of crisis intervention strategies and behavioral health services. Familiarity with child welfare systems and regulations. Strong communication skills, both verbal and written, with the ability to engage effectively with diverse populations. Ability to work independently as well as part of a team in a fast-paced environment. Proficiency in motivational interviewing techniques is a plus. Understanding of workforce development initiatives is desirable. Commitment to maintaining confidentiality and ethical standards in all client interactions.
