Outreach Case Manager Miami Recovery Project Miami, FL 33173 Job Summary We are looking for a compassionate and proactive Outreach Case Manager to join our team. In this role, you’ll work directly with individuals experiencing homelessness—providing support, building trust, and connecting them to vital housing and treatment services. This is a field-based role that requires empathy, resilience, and a strong understanding of the housing and addiction support systems. Duties Conduct street outreach to engage unhoused individuals and educate them on housing and support services. Deliver case management services in the field, including needs assessments and tailored housing plans. Provide addiction support and connect clients to recovery resources. Perform motivational interviewing to guide individuals toward stabilization. Refer clients to Continuum of Care (CoC) resources and ensure warm hand-offs to service providers. Maintain accurate and confidential documentation of outreach efforts and client progress. Assist clients in navigating complex systems including health care, benefits, and housing. Qualifications Strong communication skills, both written and verbal. Empathy and cultural sensitivity when working with vulnerable populations. Knowledge of housing programs, CoC systems, and eligibility processes. Experience or familiarity with addiction counseling and community resources. Ability to collect and analyze outreach data for referrals and reporting. Highly organized and detail-oriented in case documentation and client follow-up. Spanish language proficiency is preferred. Must be able to commute within Miami-Dade County, FL (Required).
