Job Description
Job Summary We are seeking a full-time Housing Case Manager to provide community-based support to individuals and families transitioning out of homelessness. This role is rooted in a Housing First approach and involves working directly with participants in their homes and communities to promote housing stability and self-sufficiency.
Key Responsibilities:
Provide outreach, assessment, and ongoing case management
Develop and implement individualized service plans
Facilitate housing placement and retention
Coordinate with service providers, landlords, and community partners
Maintain accurate and timely documentation
Requirements:
Bachelor’s degree preferred or 2+ years of relevant experience
Strong communication and organizational skills
Empathy and professionalism in working with vulnerable populations
Ability to transport clients in a personal vehicle
Schedule:
Monday-Friday, flexible hours between 8:30 AM – 5:00 PM This position is ideal for someone passionate about housing stability and interested in working with specialized populations, including veterans. Transferable skills welcomed-training provided for the right candidate.
