Pay:
$29.00 – $47.00/hour + $5,000 sign-on bonus!
Case Manager Opportunity:
Work with interdisciplinary team, guiding treatment plans based on patient needs and preferences. Coordinate with interdisciplinary team to establish tentative discharge plan and contingency plans Participate in planning for and the execution of patient discharge experience. Case Manager will monitor patient experience: quality/timeliness/service appropriateness/payors/expectations.
Case Manager Qualifications:
Case Manager must be a licensed RN or Social Worker Open to new grads
