Full Job Description Job Overview:
The Healthy Blue Case Manager assists Adult and Clinical Services Program Manager in the oversight of reStart’s clinical programs. This position is responsible to coordinate and provide case management and crisis services to clients that are being referred through designated referral sources. Provide oversight for service delivery and implement and monitor policy and practices in the program. The Healthy Blue Case Manager will assist with reporting to funders, maintaining daily program census records, charting and documentation, quality checks, coordination of services with service providers, and supervision of participants in the program. The Healthy Blue Case Manager will work collaboratively across programs and agencies to ensure that the program and clinical needs of participants is met.
Duties/Responsibilities:
Models positive and healthy relationship building by treating all clients, staff, and volunteers equitably with dignity, kindness, respect and Safety. Manages intake processes for each program participant, ensuring that processes for onboarding are trauma informed and welcoming Communicates with partner agencies to provide and coordinates services for all participants Monitors and supervises designated program participants Provides oversight for all program operations, including housekeeping, maintenance and participant cleaning activities Makes room assignments Processes client grievances and provides mediation as indicated Participates in the community-wide treatment team meetings Works with partner agency case managers to provide documentation and resources for placement following exit from reStart Maintains daily census tracking and monthly billing Participates in regularly scheduled Team meetings Works in collaboration with Program Manager and all other agency staff to facilitate a team environment Participates in weekly case staffing/case conferencing meetings Implements and ensures adherence to reStart policies Ensures that documentation is timely, accurate, and well written Participates in developing, implementing and evaluating plans, work processes, systems, and procedures to achieve annual goals,objectives, work standards, and grant outcomes Ensures timely data input into HMIS to meet funding requirements and statistical reporting Prepares monthly, quarterly, and annual program reports for funders,

Education and Experience:
Bachelor’s Degree in social service related field required, Master’s Degree preferred 2-5 years of experience working with people experiencing mental health and/or substance use disorders Preference given to those with experience in program development and coordination
