This person will establish supportive, professional relationships with program staff, residents, stakeholders, and families; ensuring quality of care, program implementation, regulatory compliance, and household efficiency. This person provides supervision and performance management for Empowerment Coaches and advises residents in assessing individual needs and setting goals/monitoring progress and advocates for residents’ access to community resources and opportunities in support of the agency’s vision. The Transitional Living Program consists of two cottages of six residents, one for males and one for females, with four dedicated staff. The program residents are our oldest teens and young adults, ages 16 to 20.
DUTIES AND RESPONSIBILITIES
As Supervisor Ensure TLP Empowerment Coaches provide a safe and secure environment for residents in accordance with all regulatory standards through visual observation, inspection, documentation and correction. Serve as the main point of contact for Empowerment Coaches for guidance in handling on-going issues within the cottage, new or unusual circumstances or events, and instruction during emergency situations. Provide on-call support after hours on nights, weekends, and holidays to assist Empowerment Coaches over the phone and in-person when needed. Participate in staff meetings, in-services, case reviews, and other meetings as scheduled. Provide orientation and ongoing training for Empowerment Coaches and ensure ACH policies and procedures are understood and implemented. Educate and mentor Empowerment Coaches in the utilization of CARE and TCI strategies to minimize interpersonal conflict and maintain consistent structure in the cottages. Provide formal supervision for Empowerment Coaches with one-on-one monthly staffing to promote professional growth and improved quality of care. Ensure staff adhere to daily paperwork and documentation procedures. Maintain all supervisory documentation for respective staff and program. Inspect cottages for proper facility maintenance/upkeep, acceptable inventory levels of all necessary household supplies, and correct or report issues when needed. Assist in planning social, recreational, and enrichment activities for TLP residents that teach competencies and life skills. Schedule appropriate staff coverage to maintain approved ratios and in extenuating circumstances, be willing and able to provide coverage in cottages including but not limited to overnight coverage.
As Case Manager:
Collaborate with DSS Social Workers, guardians ad litem, and other professionals or vested parties to promote a holistic team approach in the service of residents. Lead the intake process for TLP through collaboration with the Residential Services Manager to review and select candidates for admission. Secure all required information, documentation, and signatures upon acceptance into the program and at the time of intake/discharge. Responsible for measuring outcomes, record keeping/review, and maintenance of all case files with up-to-date information. Ensure access to counseling and appropriate services for residents and monitor scheduling of health care appointments. Assist residents with self-assessment of strengths and needs to create and/or update the individual service plan, set goals, and evaluate progress. Advocate for residents’ access to community resources, and coordinate educational and career opportunities. Cultivate caring, supportive relationships with residents and provide a minimum of one hour per month of individual face-to-face time with each resident. Participate in ongoing training and development opportunities to enhance your professional skill set and meet training hour requirements for your position.
