Case Manager/Adult Partial Care – Temporary Healthcare Commons, Inc. 500 South Pennsville Auburn Road, Carneys Point, NJ 08069 Job description BS/BA in Mental Health related field w/ 1 year FT exp. in mental health field OR Associates Degree in Mental Health related field with 2 years experience in human service field Must have a clean driving record Group counseling and discussion groups Implement and coordinate program Provide daily case entries that reflect program attendance Provide weekly case entries that reflect specific goals established with clients Coordinate and monitor professional or other services Provide/coordinate transportation to and from the program Monitor doctor and lab appointments Crisis intervention counseling Monitoring and supervision of consumers Spanish speaking preferred (not mandatory) Visit our website at www.hcommons.com Healthcare Commons, Inc. is a not-for-profit licensed community mental health agency serving all of Salem County for 70 years, as well as parts of Cumberland and Gloucester counties.
