WSFSSH creates and preserves affordable housing while supporting the development of healthy neighborhoods.
JOB SUMMARY
The Caseworker reports to the Director of the Center and will provide a comprehensive range of case management services to the members of the Older Adult Center.
JOB DUTIES & RESPONSIBILITIES
Responsibilities include but are not limited to: Responsible for developing and maintaining client files (written and computerized). Responsible for entering information/units/case notes into the agency database. Serve as a mediator in solving issues between clients and service providers (placing phone calls on behalf of the client). Assistance with benefits and entitlements and follow ups: HEAP applications Cash Assistance applications Access
- A•Ride applications Session 8 renewal applications Medicaid applications Medicare saving programs applications SCRIE initial, renewal and TAC adjustments forms Health Insurance plan enrollment and dis-enrollment Voter registration Assist with NYC 210 Tax Credit forms SNAP initial, renewal applications and schedule SNAP interviews Reduced•Fare MetroCard applications Schedule doctor’s appointments.
Call for Housing repairs Immigration assistance and information. NYC ID applications. Assistance with filling out money orders and checks. Assistance with medical, mental health services, and long-term care service, when appropriate. Provide information and referrals to outside agencies, where appropriate. Assistance with mail transportation and mail organization. Telephone Reassurance calls. Monthly units report.
REQUIRED KNOWLEDGE SKILLS & ABILITIES
Fluency in written and oral English required Bi-lingual Spanish a plus Good written and oral communication skills. Good organizational ability, time management and interpersonal skills. Desire or interest in working and supporting an aging population
REQUIRED EDUCATION, EXPERIENCE AND CREDENTIALS
4-year degree in Social Work with 4 years of experience working with the elderly population; OR, 8 years of Case Manager experience in lieu of degree.
PHYSICAL REQUIREMENTS
Must be able to sit and stand for long periods of time with heavy usage of computers.
