Position Description:

The Case Manager for WPA’s RISE Permanent Supportive Housing Program is responsible for providing timely, comprehensive care and support to justice-involved women and their families. Grounded in a holistic and resident-centered approach, the Case Manager addresses tenant needs, coordinates service referrals, and collaborates with internal staff and community partners to ensure consistent, high-quality programming. This role plays a key part in developing and sustaining a positive, supportive environment for tenants, staff, and the broader community by fostering respectful relationships and ensuring responsive, high-quality services. Responsibilities include conducting swift and accurate screenings of prospective tenants, maintaining ongoing follow-up, and ensuring that all applications are completed and submitted in a timely manner. This is a hybrid position requiring a minimum of three in-person days per week, with flexibility based on the program needs.

Essential Duties & Responsibilities:

Conduct a minimum of 2 monthly resident check-ins, including at least one in-person meeting
Provide direct resident services including engagement, intake, assessment, service plan development, referrals, advocacy, counseling, and follow-up
Refer residents to appropriate internal and external support services, such as education, employment, pantry, etc., to meet the goals and needs of the resident’s service plan
Develop individualized, holistic service plans that reflect each resident’s goals, needs, and strengths
Assist in building a strong community and community programming by helping to plan and host monthly social events and similar programming that fosters peer connection, engagement, and a sense of belonging
Maintain accurate and organized tenant records, and ensure all documentation is completed in a timely manner and in compliance with agency and funder standards
Respond to resident emails and inquiries promptly and address resident needs in a timely and respectful manner
Attend weekly team meetings, case conferences, and scheduled supervision session
Serve as a liaison with external providers, community partners, and funders to support coordinated service delivery
Build trusting, supportive, collaborative relationships with tenants rooted in respect, empowerment, and cultural humility
Collaborate across WPA programs to ensure coordinated, respectful, and comprehensive support for women and their families
Adhere to all applicable policies and procedures, including DHS shelter guidelines, safety protocols, confidentiality standards, and HIPAA privacy and security regulations
Participate in all required training sessions and ongoing professional development, including but not limited to trauma-informed care, housing, and social justice
Hybrid position, responsible for attending a minimum of three in-person days per week, with flexibility based on program needs. Occasional evening or weekend hours may be required to attend community events, support tenants, or participate in team activities
Perform additional duties or special projects assigned by supervisor or leadership

Qualifications:

Education/Certification Requirements B.A. in Social Work, Human Services, Psychology, or related field preferred
CPR/First Aid Certification preferred Experience Required 2+ years of documented experience in case management, preferably working with individuals who have lived-experience and/or trauma-impacted populations
Experience in Motivational Interviewing, Crisis de-escalation and Trauma-Informed Care
Experience working with individuals impacted by substance use, incarceration, HIV/AIDS, mental health challenges, domestic violence, and/or communication barriers
Familiarity with government-sponsored benefits (HRA, SNAP, SSI/SSD, etc.) and criminal justice system navigation is strongly preferred
Experience with electronic record keeping, preferably Salesforce
Bilingual Spanish/English preferred Experience Required Demonstrate ability to build trust and maintain culturally responsive, nonjudgemental communication with tenants, rooted in empathy, active listening and emotional attune
Demonstrates strong assessment and engagement skills, with the ability to quickly build trust and rapport across a diverse resident population
Contribute creative, resident-centered ideas that support a safe, welcoming, supportive and empowering environment for our residents
Exercises sound judgment and quick decision-making, particularly in high-pressure or crisis situations.
Proficient in Microsoft 365, including Outlook, Word, and Excel
Knowledgeable in trauma-informed care, harm-reduction strategies, and resident-centered practices.
Communicates clearly and professionally, both verbally and in writing,skilled in accurate documentation and case notetaking
Highly organized, detail-oriented, and able to manage competing priorities and tight deadlines
Maintains confidentiality and handles sensitive resident information with discretion and professionalism
Demonstrates cultural awareness and sensitivity to racial, gender, and socioeconomic issues impacting justice-involved women
