Community Health Worker Position Available In New York, New York

Community Health Worker 3.0 3.0 out of 5 stars New York, NY Xincon is seeking dedicated and compassionate Community Health Workers (CHWs) to join our team. CHWs will play a vital role in implementing our community-based service programs by providing in-person healthcare and social support to patients and members. Responsibilities will include promoting healthcare access, improving medication adherence, and addressing social service needs.

Responsibilities:

1. Conduct home visits and 6-month assessments across NYS to improve the quality of homecare and address additional social service needs. 2. Complete required assessment documentation 3. Follow all applicable state policies, procedures, and protocols. 4. Prepare and submit reports to address client concerns, collect feedback, and evaluate service outcomes to improve service quality if needed 5. Conduct client satisfaction surveys if needed. 6. Collaborate with other teams to ensure coordinated care and supports. 7. Perform other related duties as assigned.

Qualifications:

1. Associate degree or equivalent; CHW certification preferred but not required. 2. Strong knowledge of NYC programs and the safety net system. 3. Previous experience in community health or medical assistance is highly desired. 4. Familiarity with smartphone technology and applications. 5. Strong communication and interpersonal skills. 6. English fluency is required; bilingual ability in Chinese or Spanish is highly preferred. Additional language skills are a plus. 7. Ability to work independently and as part of a team.

Working Conditions:

This is a Per Diem position involving in-person home visits to deliver health and social services. Flexible hours based on patients or members needs and project requirements.

$40.00 – $45.00 per hour
