Associates or Bachelor’s Degree Valid NYS Driver’s License Responsibilities:
Performing a careful assessment of participants and developing and monitoring case plans in coordination with a formal assessment of suicide prevention services needed, including necessary follow-up activities, to ensure that the participant’s needs are adequately addressed Establish linkages with appropriate agencies and service providers in the area to help participants obtain needed suicide prevention services Provide holistic and comprehensive case management services to all clients to include intake assessment, benefit assessment, goal setting, long-term case management, tenant education, advocacy, and referrals. Providing referrals to participants and related activities (such as scheduling appointments for participants) to help participants obtain needed suicide prevention services, such as medical, social, and educational assistance or other suicide prevention services to address participants’ identified needs and goals Deciding how resources and services are allocated to participants on the basis of need Educating participants on issues, including, but not limited to, suicide prevention services availability Participate in other activities approved by VA, to serve the comprehensive needs of participants for the purpose of reducing suicide risk Identify opportunities for collaborative ventures with local apartment complexes, rental agencies and owners of rental house to begin the process of finding local affordable housing for Veterans Ensures all applicable regulations and guidelines are followed before requesting approval for assistance Connect families with needed and available community resources. Follow-up with clients and agencies as appropriate to document use/success of referral Help all Veterans and their families in need Attend community-based meetings when needed Work closely with the VA, DOL, and any other government agencies and community agencies as needed Complete SPOA applications for eligible clients Collect data and complete required reports
