Flexible Work Schedules Remitted Tuition Professional Development Competitive Pay and Work Life Benefits Professional Supervision and Coaching Recognition and Appreciation Programs Opportunities for Growth and Promotion Responsibilities The Health Related Social Needs Case Worker (HRSN) will help to coordinate and deliver Health Related Social Needs services to Socal Care Network Members. They will be responsible for conducting HRSN screenings, eligibility assessments, network navigation, and HRSN Care Management activities for Medicaid clients. The HRSN will act upon by HRSN need referrals within required timeframes and redirect as necessary to support service connection. The HRSN Case Worker Document progress notes and action taken with each referral, as detailed in funding source policy and procedure. Maintain effective communication with internal team members, community members, and partner organizations to ensure acceptance, resolution, or redirection of referrals. They will manage purchases and delivery of identified appliances, equipment, and other household goods to fulfill social care plan. Manage, order, and deliver food for distribution to clients as determined by social care plan. While providing tenancy support services, including but not limited to: assistance with navigating the housing application, negotiating lease agreements, and preparing for tenant interviews. This position will also provide tenancy sustaining services, including but not limited to: assistance in linking to free or affordable legal services for housing related issues, assistance with lease renewals, and housing subsidy renewals. The HRSN Case Worker will create and submit invoices for rental support, security deposits, moving and relocation services, and utility payments. They will also document progress notes and action taken with each client as detailed in funding source policy and procedure. This Position is Full Time at 35 Hours a week. Pay for the Position is $23.50 Per Hour. Qualifications Education and/or Experience Required at
Entry:
Associate degree in health, social services, or related field with three (3) years related experience in a clinical, non-profit, or Managed-Care Organization (MCO) environment. Equivalent combination of education and work experience may be considered. Thorough working knowledge of local and regional public and private resources, including Social Security, Department of Social Service Adult Protective Unit, Income Assistance Unit and various other agencies and offices as needed.
Licenses/Certificate/Clearances:
Valid N.Y.S. Driver’s License, Registered and Insured Vehicle, State Central Register Clearance.
Skills:
Intermediate computer skills, confidentiality, oral and written communication skills
Working Conditions and Environment/Physical Demands:
Mobility sufficient to drive to and participate in meetings at client’s homes and/or community facilities which may not meet Federal accessibility standards for disabled individuals, Manual dexterity sufficient to work with children/adults in a challenging and active environment.
