Case Manager/Scheduling Coordinator – Home Care 3.6 3.6 out of 5 stars 2151 Linglestown Rd, Harrisburg, PA 17110 GHG Home Care is seeking a Scheduling Coordinator/Case Manager for our growing office. We provide high-quality home care to assist the elderly and people with disabilities to live safely and independently in their own homes. What makes us different is our team oriented and personal approach to home care. A Scheduling Coordinator/Case Manager is the person who supervises and directs home health care aides that visit patients who need help in their homes with nursing, light tasks, and/or maintenance duties. The Coordinator typically helps to ensure that properly credentialed care personnel are working with patients.
Job Responsibilities:
Responsible for scheduling of aides. Conducting client assessments and home visits to assess service needs and create and help execute service plans. Attain new clients by developing relationships with Service Coordinators, current clients and working side by side with marketing teams. Monitor hours of service delivered, quality of service delivery, client satisfaction and changes in client’s functional status. Take the extra mile and interact with patients to keep track of their progress and to ensure satisfaction. Ability to travel at times to open new cases and visit current consumers. Ability to work overtime hours, take after hours calls and be on call for scheduling and
Case Management Requirements:
Bilingual in both English and Spanish is a PLUS! Able to be on call for scheduling and Case Management Proven working experience in scheduling, case management or other related job field. Excellent knowledge of case management principles. Effective communication skills Excellent organizational and time management skills Problem solving skills and ability to multi-task Basic knowledge of Microsoft Word, Excel, and Google Sheets Ability to work in a team environment Excellent follow up skills as well as attention to detail Must have a Valid Driver’s License.
