 Program Social Worker

JOB RESPONSIBILITIES

The Case Manager will meet with a caseload of 15-20 youth participants weekly. Offer one-on-one educational sessions with individuals who cannot attend group sessions. Offer one-on-one case management services to assigned participants. Facilitate and schedule case plan meetings within 7 days of participant enrollment into the program. In collaboration with the PhillyConnect Team, h/she monitors the goals and objectives in the participants’ case plan or IAP. Tracks performance indicators/outcomes for each participant. Identifies referral partners and resources for the PhillyConnect team. Complete referrals for participants to community-based needed resources (i.e., therapy, housing, educational resources; this list is not exhaustive.) Support PhillyConnect Social Worker tracking assessment scores. Submit schedule and typed progress notes weekly to the PhillyConnect Social Worker for review. Meet weekly for supervision with the Social Worker. Attend all required training courses and meetings. Pursue other training for professional development. Collaborate with PhillyConnect team members via phone, email and/or face-to-face weekly to gather information on the status of the youth’s progress (i.e., school, medical). Record all phone calls, emails, and face-to-face contact with providers in the organization’s database. Assist Youth/Mentor Advocates with getting participants enrolled in prosocial activities. In conjunction with the PhillyConnect team, S/he plans for effective programming for participants. Conduct home visits as needed/School visits Ensures that all City, State and foundation funding requirements and standards are effectively and efficiently met. Must be able to work one evening per week. May perform other duties assigned by the Program Director and/or Social Worker.

QUALIFICATIONS

Bachelor’s degree with two years of case management experience working with youth in the following fields: mental health, juvenile justice, family court or behavioral health. • Must pass the following background checks: child abuse, and FBI. •
