Position Overview:

The Community Care Liaison acts as a vital link between individuals in need of behavioral health services and River Edge Behavioral Health’s programs. This role ensures timely access to services; coordinates care with internal and external providers, and manages referral flow to optimize therapist utilization and client engagement. The starting pay for this position will be $18.00.

Location/Schedule:

175 Emery Highway, Macon, GA Monday-Friday 8:30 am-5:00 pm

Key Responsibilities:

Serve as the primary point of contact for referrals from hospitals, law enforcement, DFCS, schools, and other partners. Conduct outreach to engage referred individuals, provide program education, and assist with service navigation. Facilitate warm hand-offs and ensure timely service linkage that promotes person-centered care. Partner with crisis teams and stakeholders to support diversion from emergency departments, jails, and other restrictive settings. Represent the agency in community outreach efforts, interagency meetings, and stakeholder initiatives.

Qualifications:

Associate’s or bachelor’s degree in social work, Psychology, Public Health, Behavioral Health, or a related field (required). Minimum two years of experience in behavioral health, case management, intake coordination, or community outreach. Experience in a clinical or therapeutic setting, particularly with scheduling or utilization oversight (preferred).
