Responsibilities:

The Social Services Specialist works withnursing center patients/residents and their familymembers/significant others to identify patients/residents’strengths, social, emotional, and mental health needs and thenprocures the services to meet those needs. This position providespatients/residents with the highest practical level of physical,mental, and psychosocial well-being and quality of life. Servicesare provided in accordance with the National Association of SocialWorkers (NASW) Code of Ethics and compliance with federal, state,and local guidelines and regulations, policies and procedures, andstandards of care.

• Assists with planning and implementing acomprehensive Social Services program.
• Works with theinterdisciplinary team to promote and protect patients/residents’rights and the psychosocial well-being of all patients/residents.
• Complete a comprehensive Psychosocial Assessment for eachpatient/resident that identifies social, emotional, andpsychological needs and strengths. Assesses each patient/residentfor discharge.
• Identifies patient/resident discharge goals atadmission and documents initial discharge plan and involves thepatient/resident and family throughout the process.
• Educates staffregarding the role of the Social Services in the facility and thepsychosocial needs of the patients/residents and theirfamilies/significant others including the problems of aging anddisability as requested by the Social Services Director.

Qualifications:

• Master’s degree in social work or human servicesrequired.
• Must possess any certifications/licensures as requiredby State of employment to practice in long-term care.
