Job Description
The purpose of this position is to provide initial psychosocialevaluations, ongoing psychosocial counseling, direct casework services,bereavement services and community education, outreach and referrals.
Responsible for the delivery of varied social work services to hospicepatients and families and actively participate as a member of thehospice interdisciplinary team.
- Essential Job Functions
- Assessesthe psychosocial status of patients and communicates findings to theregistered nurse and other members of the interdisciplinary group.
- Carries out social evaluations, including family dynamics, caregiverabilities, communication patterns, high risks for suicide, neglect orabuse and plans intervention based on evaluation findings. Counselspatient and family/caregivers as needed in relationship to stress, andother identified coping difficulties. Provides crisis intervention whennecessary.
- Assesses for, and educates interdisciplinary group, on anyspecial needs related to the culture of the patient and family. Includescommunication, role of family, space, and any special traditions ortaboos.
- Maintains clinical records on all patients referred to socialwork.
- Point of Service documentation is an expectation of all clinicalpersonnel for quality care, accuracy, and communication standards. Theclinician is expected to appropriately document at Point of Service,have timely synchronization of patient data & effectively communicatewith the Patient Care Team.
- Educates patients and families on, andassists in, preparation of advanced directives.
- Provides informationand referral services for organization patients and families/caregiversregarding practical and environmental needs.
- Provides information topatients and families/caregivers and community agencies.
- Serves asliaison between patients and families/caregivers and communityagencies.
- Maintains collaborative relationships with organizationpersonnel to support patient care.
- Maintains and develops contractswith public and private agencies as resources for patient andpersonnel.
- Participates in the development of the plan of care,involving the patient and family, and attends regularly scheduledinterdisciplinary group meetings, assisting the team in recognizing theeffects of the psychosocial stresses on the symptoms of the terminalillness.
- Participates in discharge planning when needed.
- Otherduties as assigned.

Qualifications:
- Minimum of one (1) yearexperience in health care.
- Experience in a long term care settingpreferred.
- Bachelor’s degree preferred.

Current Massachusettslicensure:
LCSW or LICSW
- Demonstrates good verbal and writtencommunication, and organization skills.
- Salary Range
- :$73K
