Position Summary As a Social Worker , you will be at the heart of both welcoming new residents and guiding current residents through a smooth and informed care journey. You will assess potential admissions, coordinate with referral sources, and collaborate with interdisciplinary teams to ensure excellent clinical and social outcomes. Admissions Coordination Review referrals and assess clinical appropriateness in collaboration with the Admissions team Conduct pre-admission interviews and facilitate smooth admissions processes Obtain insurance authorizations and complete admission documentation Maintain accurate census tracking and admission logs Coordinate and lead high-quality tours for prospective residents and families Attend Utilization Review (UR) meetings and participate in marketing outreach Ensure timely communication of admission updates to internal teams Case Management Participate in interdisciplinary care planning and assessments Develop and update individualized discharge plans Educate residents and families on financial assistance options and post-discharge resources Coordinate home health services, DME, and community referrals Ensure post-discharge follow-up and satisfaction surveys Assist in verifying insurance coverage and maintaining documentation for authorizations What We’re Looking For Experience in admissions, case management, or social services within a healthcare setting Strong interpersonal and communication skills Working knowledge of Medicare, MaineCare, managed care, and discharge planning Ability to multitask and maintain documentation with precision Familiarity with HIPAA, patient rights, and person-centered care models CPR certification (required) Willingness to collaborate across departments and embrace process improvement Why Work at St. André? ✅ Meaningful Work ✅ Supportive, Mission-Driven Team ✅ Opportunities for Growth and Innovation ✅ Commitment to Person-Centered Care ✅ Competitive Compensation and Benefits Join Us!
