Required:
Bachelor’s or Master’s degree in social work and two years experience in social work in a mental health or medical setting. AuthoraCare Collective is currently seeking a Social Worker – Float. This is a full-time position; Monday – Friday 8:00am – 5:00pm.
Job Summary :
Responsible for providing intervention and assistance to the Social Work Department which is consistent with basic generalist social work practice.
This includes:
resource assessment and planning, intervention, evaluation, case management, information and referral, supportive presence/supportive counseling, case management, advocacy, community resource and client. Also included is long-term care planning to consist of LTC placement, completion of appropriate documentation i.e. FL-2s and obtaining PASRRs. Must be able to travel to, enter, and function in the homes of any patient/caregiver within the service area. Education/Qualifications:
Bachelor’s or Master’s degree in social work required. Two years experience in social work in a mental health or medical setting is required. Ability to concentrate with large volumes of work, handle pressure of deadlines, good judgment, ability to follow procedures, ability to work independently and with frequent interruptions. Communicate effectively with persons having highly diverse backgrounds and educational levels. Work effectively as a member of the interdisciplinary team.
Other:
Valid state-issued driver’s license required. Must carry automobile liability insurance at limits required by agency. Must have own transportation.
