SUMMARY OF POSITION

This position provides comprehensive school social work services to parents, students and school staff, addressing barriers that limit a student from receiving full benefit from their educational experience and responds to referrals from school administration, parents, teachers, the Student Support Team, the Special Education Committee and others by providing direct services and by assisting families in accessing appropriate community resources. Conduct a family-centered interviews with the student and his/her family to develop a community based Plan of Care using the individualized wraparound process and support the child/youth and the family in meeting the needs and objectives of the Plan of Care.

ESSENTIAL JOB FUNCTIONS
  • Provide social emotional learning and mental health supports to individual students, small groups, and large groups in the school setting
  • Increase student attendance and success by making home visits to collaborate on student and family mental health needs
  • Conduct parent interviews to acquire socio-developmental information as needed.
  • Analyze information gained through record reviews and interviews to determine environmental impacts and appropriate resource needs; integrate gained information into a written report and oral presentation. Enter and use appropriate case specific information into the data system.
  • Maintain an ongoing liaison with community agencies and other resources to meet student needs; refers parents and students to agencies when appropriate.
  • Serve on school committees as assigned relating to school health, safety and climate.

  • Coordinate and monitor services between agencies to meet child/youth clients and his/her family needs.
  • Develop cooperative working relationships with all members of the Provider Network serving the child/youth client and his/her family, while ensuring adherence to the Plan of Care.
  • Actively participates in transition plans for all students

EDUCATION, CERTIFICATION, AND LICENSURE
  • Masters Degree in Social Work, equivalent degree, or acceptable experience determined by the superintendent
  • 2 year experience, school setting preferred

Endorsement #0049 or Alt IV Letter of Intent

OTHER KNOWLEDGE & SKILLS
  • Strong verbal, writing and computer skills to work with diverse children, youth, families, co-workers, cross-agency stakeholders, and members of the public.
  • Ability to engage individuals from diverse backgrounds and cultures. Bilingual proficiency is desirable. Experience with minority or underserved populations is desirable
  • Strong knowledge of public benefit, mental health, child welfare and education systems
  • Commitment to system of care core values.
  • Ability to maintain strictest confidentiality guidelines.
  • Must be organized and able to work well under pressure meeting deadlines with efficiency and consistency while maintaining job requirements.
