Job Summary:

Coordinates and monitors the assessment, treatment planning process and follow up for each resident on our long-term care Units and Rehabilitation Unit, including with working with Residents with dementia and chronic illness. Serves as an advocate and liaison for and on behalf of residents’ rights and benefits.

Job Requirements:

Oversee a 40-bed sub-acute rehabilitation unit with support from departmental staff Complete initial admission assessments with Residents and/or their designated representatives Facilitate comprehensive care conferences and conduct psychosocial assessments Manage all aspects of discharge planning, including evaluating Resident needs and coordinating services such as community resources, home care, medical equipment (DME), and applicable government programs Maintain accurate and timely documentation in the medical record and develop individualized care plans Incorporate Resident history, mood, behavior, and family dynamics into psychosocial evaluations and care planning Serve as a liaison between internal departments and external agencies, including government organizations, assisted living facilities, DME providers, and community resource agencies Apply a general understanding of medical conditions to effectively support safe discharges and to collaborate with the interdisciplinary clinical team, including nursing, physical therapy, occupational therapy, speech therapy, clinicians, and dietitians Guide Residents in completing healthcare proxies and advance directives, and ensure they are regularly reviewed. Assist Residents and families with applications for Medicaid and other entitlements.

JOB REQUIREMENTS/EDUCATION

Bachelor’s Degree (minimum) in Social Work, Psychology, or a related Human Services field At least 2 years of supervised experience in a healthcare setting preferred Working knowledge of New York State Department of Health regulations The following experience is a plus: Supervised social work experience in a health care setting, working directly with individuals Working knowledge of New York State Department of Health regulations
