A full time Social Worker opportunity is becoming available; this position is focused on assisting residents to attain and maintain the highest possible physical, mental and psycho-social well-being. Services provided meet professional standards of social work practice, consistent with state and federal laws and regulations. Guides facility staff in matters of resident advocacy, protection and promotion of resident rights. Among the position’s responsibilities are: Completes and updates social services assessments for assigned residents as part of the comprehensive resident assessment process. Completes assigned sections of the RAI (MDS, Triggers, RAPs) in a timely manner per established schedule. Acts as Care Plan Coordinator as assigned. Works cooperatively with members of the interdisciplinary team to develop, implement and evaluate plan of care. Attenda care conferences as assigned. Communicates resident concerns and responses to interventions to interdisciplinary team members and direct care staff. Provides or arranges for social work counseling services as needed to attain or maintain highest possible mental and psycho-social well-being. Records progress notes in the clinical record including subjective findings, objective symptoms, observations of behavior, interventions provided to resident and resident’s responses to interventions. Provides information about community resources for legal, financial, mortuary and other servies. Intervenes on behalf of resident as needed. Assists with applications for benefits and procurement of servies, clothing, and personal care items from community sources outside of the facility. Evaluates assigned residents for discharge potential. Provides discharge planning services when discharge is anticipated that reflects the residents and family’s preferences for care, coordination of post discharge care and services including transportation, and how the resident will access and pay for services. Prepares discharge summary for resident’s record that is available, with consent of resident, for release to authorized persons and agencies. Assists with the preparation for the long term care suvery. Particiaptes in orientating new hires about residents’ rights, abuse definitions and prevention.

Qualifications include:
CDC COVID-19

Vaccination card showing fully vaccinated. Bachelors degress from an accredited school or program in Social Work or Social Welfare. Current Socail Work license preferred. One year supervised social work experience working directly with individuals in a skilled nursing faciltiy providing long term care and/or rehabilitation services.
