Position Summary/Department Summary:
We are seeking acompassionate and skilled Clinical Social Worker to join ourmultidisciplinary team in the Home Parenteral Nutrition Programwithin the Gastroenterology Department. This is a 20-hour positionwhich involves providing a range of clinical social work servicesand case management to patients with complex medical needs. Thisrole requires specialized support and comfort in areas includingparent coaching, child protection concerns, school advocacy,chronic illness adjustment, and family dynamics. The idealcandidate will bring strong clinical judgment, emotionalresilience, and a nuanced understanding of how psychosocialstressors intersect with pediatric medical conditions.
KeyResponsibilities:
- Conducting psychosocial screenings andcomprehensive assessments to guide care planning.
- Offeringlongitudinal case management—facilitating access to community andhospital-based supports, tracking care plans, and addressingpsychosocial barriers to follow-through.
- Helping familiesnavigate the intersection of chronic illness and educationalsystems and collaborate with schools to implement recommendedaccommodations and support services
- Collaborating closely with amultidisciplinary team including physicians, nurses, psychologists,and other allied health professionals.
- Fostering effective teamdynamics by facilitating open communication and collaboration amongmultidisciplinary medical teams.
- Delivering crisis intervention,safety planning, and resource mobilization as needed
- Addressingchild protection concerns in collaboration with internal andexternal agencies
- Contributing to program development andparticipate in ongoing quality improvement efforts
MinimumQualifications Education:
- Master’s degree in social work
Experience:
- Exceptional verbal and written communication andorganizational skills.
- Flexibility and ability to work in afast-paced, dynamic environment.
- Strong clinical experience inmedical or pediatric settings ideal
- People of diversity stronglyencouraged to apply.
- Spanish/Portuguese speaking preferred.
Licensure/Certifications:
LCSW in the state of MA required; LICSWpreferred This position is eligible for part-time benefits and a$2,500 sign on bonus!
