Targeted Case Manager 2.5 2.5 out of 5 stars Polk County, FL Serving Children and Reaching Families, LLC is seeking a Targeted Case Manager to provide Outpatient Mental Health services to adults and adolescents in need. The ideal candidate will have a strong background in mental health services and a passion for working with diverse populations.
Position Details:
1099 Contract (No benefits provided)
Pay:
$23.00 – $27.00 per hourFlexible schedule, including morning, day, evening, and weekend shifts as neededHybrid position with 50% travel requiredRemote meetings and trainingsVirtual interview process
Required Knowledge and Abilities:
Applicant must have applicable knowledge of Medicaid rules for Targeted Case Management service delivery when providing services to Medicaid-enrolled clientsMust have knowledge of and apply recovery-based principles and practices in service delivery that includes addressing and documenting the client’s needs, strengths, abilities, barriers, and preferencesMust be knowledgeable of community resourcesMust be able to work with individuals who have a mental health disabilityHas completed the CBHCM certification provided by the Florida Certification Board or currently holds a provisional certification.
Must be reliable and a team playerAbility to provide services in the client’s home, or in other community settingsAbility to attend virtual team meetings, supervision meetings, and trainings according to agency needsApplicant must have equipment to perform job duties and a stable internet connectionHas knowledge of psychiatric diagnosis and ability to recognize signs and/or symptoms of psychiatric illness
Responsibilities:
Conducting assessments to determine client needs and eligibility for servicesDeveloping individualized treatment plans in collaboration with clientsMonitoring client progress and adjusting treatment plans as neededAdvocating on behalf of clients to ensure they have access to necessary resources and supportLinking clients to community resources and services to support their mental health goals
Qualifications:
