What You’ll Do:
Assess needs and create personalized support plans Coordinate services and community referrals Provide ongoing support and crisis response Advocate for individual rights and well-being Maintain clear documentation and track progress Participate in on-call and some weekend coverage
What You Bring:
Bachelor’s degree in social work, psychology, or a related field OR 12 relevant credits plus 2 years of experience Valid driver’s license, reliable transportation, and insurance Experience in mental health or social services preferred
