We are currently seeking a compassionate, dedicated Social Worker to join our team and make a meaningful impact in the lives of our residents.
Job Description :
As a Social Worker, you will work directly with residents and their families to provide social services and emotional support, ensuring a high level of care and well-being for each resident. You will collaborate with the interdisciplinary team to create personalized care plans and assist with discharge planning, advocacy, and crisis intervention.
Social Worker Responsibilities :
Assess residents’ psychosocial needs and work with families to address concerns. Develop, implement, and update individualized care plans in collaboration with the care team. Provide counseling and support services to residents and families. Assist with discharge planning, ensuring smooth transitions and appropriate community referrals. Advocate for residents’ rights and ensure compliance with facility policies and regulatory standards. Participate in care plan meetings and other interdisciplinary team meetings. Maintain accurate and timely documentation in compliance with facility and regulatory standards.
Social Worker Qualifications :
Bachelors degree in Human Services field Previous experience in a skilled nursing or long-term care setting. Strong communication, organizational, and problem-solving skills. Ability to work independently and as part of a team. Empathetic, compassionate, and committed to enhancing the quality of life for residents.
