Job Description
•$1,000 sign-on bonus!•Kennebec Behavioral Health, established in 1960 to provide comprehensive behavioral health services, named one of the Best Places to Work in Maine for the last 5 years, and now is proudly serving as a Certified Community Behavioral Health Clinic ( CCBHC ), currently has openings for full-time Access Center Care Coordinators for our Skowhegan Clinic. The Access Care Coordinator provides care coordination services to child and adult clients enrolled in awaiting assignment to a KBH program, to prioritize clients with high needs and to support transitions between levels of care. The Access Center Care Coordinator provides direct care coordination services by triaging and providing initial response to meet clients' immediate needs for safety, coordination of timely follow-up by the clients' current providers and facilitating to meet acute needs. The candidate will also participate in the process of care coordination by participating in Care Coordinator supervision, collaborating with Access Center and other KBH program staff to address gaps and interruptions in care, and to facilitate transitions between services. The Care Coordinator will also document client care as evidenced by accurate medical records by completing documentation in accordance with KBH guidelines, responding to clinical and administrative audits in accordance with KBH guidelines, and follow the ethical standards set forth by Licensing standards and in accordance with KBH guidelines
Requirements:
Bachelor's degree in Human Services related field, and eligible for MHRT/C. Four years of care coordination experience a plus. Knowledge of medical and behavioral health systems of care, strong computer skills, communication, and customer service skills are preferred.Benefits:
