Job Description

Parktree Community Health Center provides comprehensive and integrative medical, dental, and behavioral health services to the local community in the Pomona and Ontario areas. We are looking for an energetic and professional Outreach and Enrollment Specialist to join our team.
Responsibilities include:
Initiating contact with newly assigned managed care patients, initiating appointments to establish enrollment within the health center. Complete initial patient paperwork required such as; financial screening documents, sliding fee scale application, other program applications. Ensure accurate and complete payor set up within the patient's chart are completed upon enrollment within ParkTree Community Health Center. Provide educational guidance for both patients and staff regarding programs and services provided at Parktree Community Health Center. Maintain accurate documentation and tracking of all enrollment efforts including to but not limited to; meeting scheduling metrics set by Director of Revenue Cycle Operations, maintaining current records of managed care rosters, reporting outcomes on a weekly/monthly basis to Director of Revenue Cycle of Operations. Collaborating with all departments to ensure quality goals are met as defined by leadership. Advocate for patients by assuring the best use of program eligibility defined by services provided by specific sites. Maintain current knowledge of various program/payor requirements and eligibility guidelines as well as reporting updates and changes of all accepted programs to appropriate departments and department leaders. Engage with all patients and staff in professional demeanor, remaining a good steward of company resources identifying and reporting areas of improvements to leadership in a timely manner. Adhere to HIPAA regulations maintaining patient confidentiality at all times. Participate in community outreach events and maintain professional relationships with community partners.
Qualifications:
Bilingual in both English and Spanish required High School Diploma or equivalent required Ability to work with diverse patient populations in a respectful manner Knowledge of EHR/EPM systems, basic computer skills including basic understanding of MS Word, Excel and Outlook preferred NextGen experience Knowledge of Medi-Cal Managed Care plans, and various Medi-Cal programs preferred Understanding of all contracted payors, including Medicare and Medicare Managed Care plans preferred Ability to understand and maneuver various payor portals Develop professional relationships with provider representatives of contracted payors Possess effective communication skills, both written and verbal formats Strong critical thinking and problem solving skills Effective time management skills Ability to effectively manage competing priorities and meet deadlines as requestedStrong customer service skills Comprehensive Employee Benefits including: Comprehensive Medical/Dental/Vision benefits for Full-Time employees Paid holidays, vacation and Sick time Retirement Plan with employer contribution Tuition Assistance Continuing Education benefits • Opportunity to apply for
