Job Summary:
As an Enrollment Representative, you will serve as the primary point of contact for prospective participants and caregivers throughout the intake and enrollment process. You will educate individuals on available services, coordinate Medicaid and enrollment documentation, and build strong referral relationships within the healthcare community. This role combines healthcare outreach, consultative sales, and patient advocacy to help seniors successfully access care and services that allow them to age safely in place.
Key Responsibilities:
Manage qualified referrals from hospitals, providers, brokers, and community referral sources Conduct intake appointments, home visits, tours, and enrollment meetings Educate prospective participants and caregivers on PACE program services and benefits Coordinate collection of required enrollment and Medicaid documentation Maintain accurate and real-time updates within CRM systems Collaborate with interdisciplinary teams, operations, and eligibility departments throughout the enrollment process Build and maintain referral relationships with healthcare providers, senior living communities, and community organizations Present participant cases to enrollment decision teams Utilize consultative sales techniques to guide and close enrollments Ensure a positive onboarding experience for all new participants
Qualifications:
2+ years of healthcare sales, admissions, intake, enrollment, or business development experience required Experience working with both referral sources (B2B) and patients/families (B2C) preferred Background in Medicare Advantage, Managed Medicaid, home health, hospice, senior living, PACE, or related healthcare settings strongly preferred Experience using CRM systems such as Salesforce, HubSpot, Zoho, Microsoft Dynamics, or similar platforms Bachelor's degree preferred; equivalent healthcare experience considered Strong communication, organization, and relationship-building skills Valid driver's license, reliable transportation, and current auto insurance required
