Job Description:
Collects and records demographic, financial, and clinical information related to hospice referrals and admissions. Interacts and collaborates with healthcare professionals, community referral sources, patients, and families. Assists in the assignment of admissions staff to ensure timely admission response. Inputs referral data into the system accurately and efficiently. Understands end-of-life issues and care to provide appropriate support and information.
Knowledge, Skills, and Abilities:
  • Community referral sources [Required]
  • Medicare Hospice criteria [Required]
  • EMR [Required]
  • Medical Terminology [Required]
  • Hospice COPs [Preferred]
  • ICD 10 [Preferred]
  • Medical insurance billing [Preferred]
Education:
  • Associate [Preferred]
  • High School Grad or Equiv [Required]
Field of Study:
  • graduate or GED
  • in business, office systems, health care or related field
Work Experience:
  • 1+ experience in the health care field [Required]
  • Hospice or Home Health intake experience [Preferred]
Additional Information:
    N/A Licenses and Certifications:
    • Licensed Practical Nurse (LPN) [Preferred]
