Job Description
PACE, a member of Empath Health is seeking a meticulous and goal-driven Enrollment & Outreach Coordinator to join our interdisciplinary PACE team in Pinellas Park, FL . In this role, the PACE Enrollment & Outreach Coordinator accountable for coordinating events in the community and in the centers, providing tours within the center for potential participants, families and caregivers, assisting the Outreach and Enrollment with various tasks including, scheduling the PACE eligibility and enrollment intake process, creating outreach flyers for community and in-center events. Build long-term connections with participants, supporting them through all stages of care—this is a meaningful alternative to traditional inpatient or facility-based care.Team-Driven, Preventive Focus:
Collaborate closely with physicians, social workers, therapists, and other clinicians in a proactive care model focused on prevention, wellness, and holistic well-being. Answer calls from prospective participants and conduct follow-up calls to referrals, while providing great customer service, explaining the Empath Health PACE enrollment process, and converting all inbound inquiries to home visits or tours. Run Medicaid eligibility for PACE enrollment, and when needed, assign possible enrollments to the Medicaid team for application support. Demonstrate in-depth knowledge of PACE. Schedule of initial home visits, tours, intake visits, and any other pre-enrollment visits necessary as well as coordinating transportation to the center for pre-enrollment visits and/or tours. Coordination of community events and in-center events, including creating necessary flyers and ordering essential marketing materials. Aid VP of Business Development and the Outreach and Enrollment team, with meeting facilitation, using available A/V to document tasks, follow-ups, notations, and assignments of team in current CRM. Assist with PR, marketing, outreach, and enrollment projects as needed.Earn Competitive Pay:

Associate's degree in a relevant field, bachelor's degree preferred Two (2) years of relevant experience preferred Strong customer service orientation Ability to work independently with minimal supervision Reliable means of transportation Experience in CRM software is preferred.
