Full Job Description Job Description:
Hospice Accreditation Consultant (CMS / CHAP / ACHC)Alpha Comfort Care , a licensed hospice provider, is seeking an experienced Hospice Accreditation Consultant (Independent Contractor) to lead and support the organization through successful hospice accreditation in alignment with CMS Conditions of Participation (42 CFR Part 418) and recognized accrediting body standards, including CHAP and/or ACHC . Engagement Purpose The consultant will provide expert guidance to ensure Alpha Comfort Care is fully prepared for hospice accreditation, including operational, clinical, documentation, and quality compliance required to meet CMS‑deemed status through CHAP or ACHC. Scope of Services The consultant will be expected to: Conduct a comprehensive accreditation readiness assessment against CMS Hospice CoPs and applicable
CHAP/ACHC
standards. Review and align policies, procedures, and clinical documentation with regulatory and accreditation requirements. Support development and implementation of QAPI‑compliant quality assurance and performance improvement activities . Identify compliance gaps and provide clear, actionable corrective action recommendations. Assist leadership with survey preparation , mock surveys, and accreditation submission readiness. Provide technical assistance throughout the accreditation process through survey completion. Required Qualifications Demonstrated experience working directly with hospice organizations that have successfully achieved accreditation through CHAP and/or ACHC.
Strong working knowledge of:
CMS Hospice Conditions of Participation (42 CFR Part 418) Hospice QAPI requirements Accreditation survey processes and documentation standards Proven ability to guide hospice agencies from pre‑accreditation through survey completion. References Requirement Contractor must submit two (2) professional references from hospice organizations where they directly supported and successfully achieved accreditation. Engagement Type Independent Contractor / Consultant Project‑based, accreditation‑focused engagement
Experience:
Hospice care: 3 years (Required)
License/Certification:
LPN (Preferred)
