$42,209364 - $58,837.68 per year How you'll make an impact in this role: Drive participant enrollment for the Program of All-Inclusive Care for the Elderly (PACE) by managing the full enrollment process while maintaining program capacity goals. Build and maintain strategic partnerships with hospitals, home health agencies, hospice providers, and community organizations to generate referrals and support participant transitions. Lead community outreach, educational presentations, and special events to increase program awareness, strengthen community engagement, and expand referral networks. Monitor enrollment metrics, maintain accurate reporting and statistical data, and identify opportunities to improve participant acquisition and retention. What minimum qualifications you'll need:
Education:
High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Bachelor's degree OR 4 years of applicable cumulative job specific experience required Life at
