Job Description

POSITION RESPONSIBILITIES
Visit hospitals, nursing homes, assisted living, and clinics. Maintain strong relationships with doctors, nurses, social workers, and discharge planners. Represent the hospice professionally in the community. Provide education about hospice benefits and eligibility. Answer common questions about insurance, medications, equipment, and eligibility. Coordinate with admissions, nursing, and the office team to start services. Explain the referral process to families and providers.
QUALIFICATIONS 1.
Hospice liaison experience preferred. 2. Must be organized and detail-oriented. 3. Must be able to communicate effectively orally and in writing.
