Job Description
OUTREACH SPECIALIST
Meals On Wheels of Wake County- 4.
PURPOSE OF POSITION
Responsible for assessing needs and eligibility for applicants of Meals on Wheels (MOW) of Wake County to ensure the neediest clients are served efficiently and effectively. Ensure all program requirements are met by regular assessments and client visits.MAJOR RESPONSIBILITIES
Visit and screen new applicants to determine level of need and eligibility. Refer to appropriate Meals on Wheels program or other agency based on need and eligibility. Visit new Home and Community Care Block Grant (HCCBG) recipients to gather client information for files and to distribute information about the program. Prepare documentation to ensure recipients are properly enrolled in the program. Reassess all HCCBG recipients every six (6) months to ensure eligibility requirements are met. Provide accurate reports of ongoing assessment so that block grant requirements are satisfied. Develop and maintain excellent working relationships with other related agencies to identify resources that would meet MOW participants' needs. Conduct research and studies as needed to ensure MOW stays abreast of trends on nutrition and aging. Represent MOW in the community as necessary. Assist with special projects and other duties as assigned.REPORTS TO

Valid Driver's License and reliable transportation Bachelor's degree or equivalent experience, preferably in Social Work Strong organizational skills Excellent written and verbal communication skills Strong interpersonal skills; ability to develop and maintain effective working relationships with all constituencies Strong analytical skills; ability to gather and analyze data Ability to work independently and prioritize work based on MOW's needs and priorities Ability to coach and teach Verifiable computer knowledge: ability to use MS Windows, MS Word, MS Excel; email; client management software; and other technology as appropriate Assist with meal delivery as neededEnvironment:
Combination of interior and outdoors; may occasionally work in inclement weather conditions.Physical Demands:
