Job Description

Coordinate the intake & assessment process for new participants Join Our Team as an Intake/Enrollment Coordinator! Are you passionate about helping seniors access the care they need ? Northland PACE Senior Care Services is looking for a dedicated and detail-oriented Intake/Enrollment Coordinator to guide seniors and families through the enrollment process and promote our life-changing program!
What You'll Do:
Conduct in-home, nursing home, and hospital assessments Maintain detailed records and determine funding sources ( Medicaid, Medicare, private pay ) Engage in marketing & outreach to connect with the community Why Join Us? Make a Difference - Help seniors receive quality care in their community Supportive Team - Work alongside a compassionate healthcare network Competitive Pay & Benefits Must have: Minimum education, Associates Degree. Experience may substitute for educational requirement Minimum of two years direct service experience in a healthcare setting Proof of a valid driver's license, good driving record and auto insurance as required by state law
