Job:
The Intake Support Specialist plays a critical role in coordinating patient referrals and ensuring a smooth intake process for home health and hospice services. This position serves as a key point of contact for referral sources, patients, and internal teams, ensuring accurate information, timely processing, and excellent customer service.
What You'll Do:
Manage incoming referrals and ensure accurate, timely processing Educate callers on agency services and connect them to appropriate resources Verify insurance benefits, authorizations, and payer requirements Create and maintain patient records in the EMR system Identify missing documentation and follow up with referral sources Collaborate with care teams, liaisons, and departments to coordinate admissions Route referrals appropriately to support timely start of care Deliver exceptional customer service to internal and external partners Maintain productivity standards and contribute to a team-oriented environment
Who You Are:
Detail-oriented with strong organizational and critical thinking skills Effective communicator (written and verbal) with a customer-first mindset Collaborative team player with a positive, solution-focused attitude Comfortable managing multiple tasks in a fast-paced environment High school diploma required; healthcare or medical office experience preferred Familiarity with EMR systems and insurance verification is a plus Meets background check requirements
