- Field Based) Join a Mission-Driven Team Supporting New Yorkers in the Community We are seeking compassionate, organized, and highly professional Intake Specialists to conduct in-person member enrollment visits and assessments throughout assigned communities.

- Conduct face-to-face intake visits (remote intake is not permitted)
- Complete outreach and pre-visit documentation
- Conduct comprehensive assessments, including:
- Medical
- Behavioral health
- Psychosocial
- Social determinants of health
- Obtain required consent and care plan signatures
- Verify supporting documentation including:
- Identification
- Insurance information
- Referrals and supporting records
- Educate members on program services, rights, and responsibilities
- Identify urgent concerns and appropriately escalate when necessary
- Complete timely and accurate post-visit documentation Documentation & Compliance
- Ensure all enrollment packages are complete, accurate, and compliant
- Enter required information into EHR/care management systems
- Maintain high standards of documentation quality and timeliness
- Follow organizational workflows, compliance protocols, and confidentiality standards Additional Field Visits
- Conduct in-person renewal visits, including:
- Signature renewals

renewals- Ensure all renewal documentation meets compliance requirements Compensation This is a per-diem, per-assignment position and is not hourly-based. Compensation is tied to successfully completed and compliant documentation:
- $250 per completed intake package
- $100 per completed renewal/signature visit Travel Reimbursement Travel expenses are reimbursable in accordance with company policy, including:
- Mileage reimbursement
- Public transportation costs
- Approved travel-related expenses Qualifications Required
- High School Diploma or GED
- Comfortable with local travel and community-based work
- Strong organizational and communication skills
- Ability to work independently and manage assignments responsibly Preferred
- Associate's or Bachelor's degree in:
- Human Services
- Social Work
- Public Health
- Healthcare Administration
- Related field
- Experience in:
- Intake coordination
- Healthcare enrollment
- Care management
- Social services
- Community outreach
- Experience completing assessments or healthcare documentation Ideal Candidate The ideal candidate is:
